Job Title: Registered Nurse Case Manager

Case Manager Registered Nurse (RN)

Position Summary: Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues will also be monitored for and reported to the QRM department as a part of the CQI process.

Position Requirements:
* Registered Nurse, Colorado Licensure. BLS certification.
* Graduate of an accredited school of nursing; BSN preferred.
* 3-5 years recent clinical or case management experience in area of specialization.
* Requires excellent human relations and oral|written communication skills. Must have the ability to asses’ information, apply critical thinking processes to determine course of action, define expected outcomes, and evaluate effectiveness of plan through continuous assessment of the patient's progress. Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews. Must also have strong analytical and organizational skills.

Minimal supervision: Individual proceeds on his|her own initiative in compliance with policies, procedures and practices prescribed by immediate supervisor.

Orientation Period: 90 day probationary period. 2 –3 weeks training|orientation contingent upon experience. Extensive orientation may be required and will be determined by the Department Director.
