As a Clinical Pharmacist, you'll play a crucial role in optimizing medication therapy, and working collaboratively with physicians, nurses, and other healthcare professionals to enhance patient outcomes. Key Responsibilities: •Review and manage patient medication plans, ensuring safe and effective use of pharmaceuticals. •Provide expert advice to healthcare professionals on medication therapies, including dosing, side effects, and drug interactions. •Counsel patients on proper medication use, potential side effects, and adherence strategies. •Work closely with physicians, nurses, and other healthcare team members to optimize patient treatment plans. •Conduct thorough medication reviews during patient admissions, transfers, and discharges to prevent errors. •Ensure compliance with all relevant pharmacy laws, regulations, and standards. •Participate in initiatives aimed at improving medication safety and patient outcomes. Pharmacist Qualifications: •1 or more years of Clinical Pharmacist experience is required. •PharmD or RPh from an accredited school. •Current and valid license to practice. •Understanding of DEA and FDA health regulations.
