Job Title: PT - Home Health

(Only QUALIFIED Healthcare Professionals accepted) Physical Therapist - General job duties. Types of procedures required.
Evaluates patient' condition and|or disability, by selecting and administering appropriate tests and measures; formulates and administers a treatment plan; interprets testing and treatment to staff, patient, family; determines need for referral to other specialties and|or agencies; maintains proper records and reports.

Required skills and minimum years of experience.
Minimum of 1 year experience as PT inpatient or outpatient. Home health experience also preferred

Specific licensures and certifications|registrations required. (ARRT, CRT, BLS, ACLS, etc)
Must have Bachelors, Masters or Doctorate degree in physical therapy. Must hold or be eligible to receive a physical therapy license in the state of Indiana from the Indiana Professional Licensure Board. Must have valid state driver's license, reliable transportation, and proof of insurance. Must have current CPR certification.

Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)
Hours can be flexible (willing to do 4 – 10 hour days 8am-7pm or 5 – 8 hour days 8am-5pm). There is a possibility for weekends so I would say no more frequently than every third weekend. There is no call.

Floating to other units|departments. (If so, to which units|departments?)
They would need to be able to float to our entire coverage area. (Ft Wayne, Huntington|Whitley, Noble)

Tell us about the unit|department specific training and orientation program.
They will attend New Hire Orientation on Monday 7:30am-12:30pm then Patient Care Orientation on Tuesday 8:00am-4:30pm.

Additional Information:

1. Required to have current valid driver’s license
2. Drivers having over 10 points as assigned by the Bureau of Motor Vehicles will be unacceptable.
3. Drivers having a conviction for operating under the influence within the last three years will be unacceptable.
4. Drivers having a conviction of reckless driving within the last three years will be unacceptable.
5. Drivers having a speeding violation in excess of 20 mph within the last three years will be unacceptable.
6. Drivers with more than two speeding violations within the last three years will be subject to further review.
