Join a team of 300+ employees who are dedicated to improving the health & well-being of the community in which they live and serve.

The Primary Care Nurse Practitioner will work full-time (40 hours|week) with his|her time split between the Norwich and New London centers. The Nurse Practitioner will provide primary care services 20 hours per week in the Norwich health center and 20 hours will be spent providing integrated primary care to Behavioral Health Clients in the New London health center.

The Nurse Practitioner Conducts medical evaluations and formulates accurate diagnoses in accordance with the International Classification of Diseases. Manages prescribed medication, performs physical health examinations, conducts preventive screening and care, develops and presents clear clinical impressions and recommendations that are specific and attainable.

Develops and documents clear clinical formulations and medical treatment plans. Prescribes therapeutic interventions appropriate to identified problems. Administers drugs and biologicals as required by the Clinic.

Willingness to obtain MAT Data Waiver is required to become part of the Medication Assisted Treatment program for primary care patients. Must be proficient seeing both adults and children in an outpatient, primary care setting.
