Summary:
Under the supervision of the director|manager or designee, the Registered Nurse provides patient care in response to identified needs.  Additional functions will be performed in accordance with the Nurse Practice Act in a manner consistent with the mission and goals of the organization.
Coordinates and oversees all direct care patient services provided by clinical personnel. 
Responsible for ensuring that patient care is coordinated and managed appropriately in a cost effective and financially responsible manner. 
Assist with development, implementation and maintenance of the Company’s quality improvement program and monitors compliance with both internal standards and regulatory requirements and adjusts clinical programs accordingly.
Responsible for the clinical nursing services of the Home Health | Hospice program and works in conjunction with other personnel, such as billing and medical records, to ensure optimal management and consistency for all aspects of patient care.
Teaches home health | hospice philosophy including compassion and clinical expertise to employees
Holds staff accountable to current clinical standards of practice and policies and procedures.
Utilizes team building skills to promote optimal team performance and support.
Utilizes problem solving skills in maximizing staff effectiveness, efficiencies and cost effectiveness.
Responsible for mentoring and providing leadership and direction to staff, monitoring and tracking statistical data as it relates to the area of responsibility
 
Education:
Associates of Nursing required, or Nursing Degree from an Accredited Nursing School
Bachelor’s of Science in Nursing and|or Masters in health care related field preferred
 
Certification | Licensure:
Current State Registered Nurse (RN) License
Cardiopulmonary Resuscitation (CPR)
Basic Life Support (BLS) preferred
Advanced Cardiac Life Support (ACLS) preferred
 
Experience:
3-5 years nursing experience required
1-2 years in a home health|hospice management role required
 
Special Skills:
Previous knowledge of or experience with Medicare, OASIS and CHAP preferred.
Must be able to utilize independent decision-making skills in a wide variety of situations.
Requires a high degree of accuracy in the performance of varied responsibilities. 
Personal computer skills required.
Frequent contact with employees, physicians, customers, community individuals and volunteers.
Must be self-motivated to complete work in a timely manner to meet deadlines.
