Job Title: Nurse Practitioner Vascular

Lynwood (Only QUALIFIED Healthcare Professionals accepted) Cardiology-NP - Under the supervision of a vascular surgeon, the Physician Assistant will be responsible for the inpatient and outpatient care coordination, direct patient care of the vascular patients. Work in an outpatient clinical practice and provide inpatient care. Participate in patient and family education. Provide leadership by working cooperatively with outpatient clinical staff and hospital clinical staff in the acute care setting. Maintain standards for professional practice in the clinical setting.

Duties
• Maintain accurate and complete medical records with appropriate documentation in hospital and outpatient electronic medical records.
• Interpret incoming lab and diagnostic radiology results
• Provide medicine reconciliation
• Provide patient and family education
• prepare and review patients’ medical records.
• Develop treatment plans for patients under the supervision of an attending physician.
• Assist in the preparation and maintenance of surgery consent forms
• Conduct patient pre-operative assessments
• Confirm all pre-operative evaluations are complete prior to surgery.
• Provide communication with patients and their family members regarding disease processes and treatment plans.
• Act as liaison between attending physician and patients and their family members.
• Collaborate with other caregivers to ensure continuity of care.
• Manage telephone inquiries from patients regarding urgent clinical issues and addresses patient questions and concerns between visits.

Minimum Requirements
• Completion of an accredited program for Physician Assistant.
• Meet credentialing requirements through Medical Staff Office
• CPR|ACLS certified
Job Type: Full-time. Monday through Friday from 9:00 am to 5:00 pm.
