SALARY - $230,000 annual base salary $230,000

Per Week of On-Call Coverage - $3,300 (COVERAGE IS NOW SCHEDULED PER WEEKDAY ($800 per 24 hour period) AND THEN FOR WEEKEND COVERAGE ($2,500 Friday 8:00 A.M. to Monday 8:00 A.M.)

Two Full Weeks/Month $79,200

One Full WeeWMonth $39,600; Average for three psychiatrists $57,200
All Sites Certified for National Health Service Corp (Loan Repayment Program)
We agree to: Moving expenses
Provide a $20,000 life insurance policy at no cost to employee.
Provide long-term disability coverage at no cost.
Provide health and dental insurance coverage for employee and family. (Emp/Spouse - $14,663, Employee Only $7,130)
Pay professional liability insurance premium for an individual policy.
Pay State Medical Board license fees and professional membership dues.
Pay DEA Registration/Renewal fees.
After one year of employment, participation in the Agency's retirement plan, which is a contribution of 4% of employee gross salary. Employee is fully vested after six years of employment. ($275,000 cap for 6/30/2019)
After one year of employment, employee physician will be eligible for twenty (20) days of vacation. The twenty (20) days of vacation are to be taken between anniversary hire date and the next succeeding anniversary hire date.
Any CME's the physician desires to obtain will be secured using time allotted within the above specified vacation days.
Sick leave accrued at the rate of 8 hours per month. Sick leave time is available to be used to cover absences of employee illness or healthcare visits. Sick leave may be accumulated up to a maximum of 90 days.
Ten (10) paid holidays per year.
