(Only QUALIFIED Healthcare Professionals accepted) Family-NP - A Federally Qualified Health Center’s (FQHC) operating as a Patient-Centered Medical Home (PCMH) is seeking a full-time primary care Nurse Practitioner|Physician Assistant.
The Nurse Practitioner|Physician Assistant will provide quality health care and treatment to patients. Perform assessments, order tests, review test results and development a plan of care for a wide variety of acute and chronic conditions.
The NP|PA provides health promotion, prevention of disease and disability; identification of health problems; management of health problems; and referral with communication to the patient's primary care provider. The Nurse Practitioner|Physician Assistant will practice medicine within the scope of their licensure and, under the Supervision of a Physician.
