Job Description
A Nurse Practitioner is needed in Canton, OH. The primary focus of the Nurse Practitioner is to deliver in-home care to geriatric patients of the community who have chronic and acute illnesses.
Details:
• Full-time, direct hire position
• Monday through Friday, 8 am to 5 pm
• Local travel to patient homes; travel on-site for in-person team meetings when needed
• Average patient volume: 8 to 12 per day
• Provide primary care to geriatric patients including initial screenings, physical examinations, and determine necessary treatment plans

Qualifications
• Valid Ohio Nurse Practitioner license
• Experience in a family practice, geriatric, ICU, or ER setting
• Experience in palliative care and end of life discussions
• Home care experience a plus
• BLS and ACLS certification
