Location Requirement: Candidates must reside in Brooklyn and have UAS assessment experience.
Job Type: Field Based, M-F, 8 am to 5 pm.
Responsibilities:
This is a field based position visiting members in their homes to do face to face assessments.
Conduct UAS assessments with members, caregivers, or providers to gather information for care plan development.
Monitor service delivery through in-person visits and telephonic contact.
Authorize and coordinate referrals for services.
Ensure seamless provider services and identify deficiencies in care plans.
Assist in integrating informal or voluntary services into member care plans.
Collaborate with discharge planners, physicians, and others for appropriate discharge and care plans.
Assist members in filing and resolving complaints and appeals.
Direct care to participating network providers.
Participate in care management committees and work on special projects related to care management.
Education|Experience:
3+ years of care management experience preferred.
Home health, discharge planning, or long term care experience preferred.
Licenses|Certifications:
Valid driver's license.
NYS RN license required.
