**Role Overview**
Plans and delivers individualized physical therapy programs while collaborating with an interdisciplinary team. Reports to the Clinical Coordinator of Rehab Services.
**Key Responsibilities**
* Evaluate patients and develop treatment plans
* Administer therapies (exercise, gait training, modalities, etc.)
* Educate patients on treatment plans and assistive devices
* Adjust care plans based on progress; consult with physicians as needed
* Complete all required documentation (evaluations, progress notes, discharges)
* Communicate with care teams to coordinate treatment
* Supervise support staff and assist with department operations
* Maintain safety, infection control, and quality standards
* Participate in meetings, training, and ongoing education
* Provide care across all age groups
* Assist with patient transport and cover senior therapist duties as needed
**Requirements**
* NYS Physical Therapy license (or eligibility at hire)
* Hospital experience preferred
* BLS required within 90 days
**Physical Demands**
* Lift 30 lbs independently, up to 150 lbs with assistance
* Regular physical activity including lifting, pushing, pulling
