(Physician/MD qualifications required) Internal Medicine - Seeking a Board Certified Internal Medicine physician to join a practice working with patients in all levels of post-acute care including LTAC, sub-acute rehab, and inpatient rehab.
Full-time position
Monday – Friday. Flexible schedule. Set your own schedule.
Responsibilities:
• Work alongside pharmacists on medication management
• Complete Medicare required visits
• Complete acute, follow up, and discharge visits as necessary
• Implement an individualized plan of care for patients' goals
• In collaboration with the IDT determine transitional care
• Make recommendations on DME, home health services, or outpatient services after short stay.
Will not accept visa candidates.
