[] Job Description: Nurse Practitioner
We seek skilled Nurse Practitioners or Physician Assistants passionate about impacting patients' lives. As an Advanced Practitioner, you will play an integral role in a Care Team alongside a Registered Nurse and a Social Worker, supporting patients with complex health needs, primarily chronic kidney disease (CKD), end-stage renal disease (ESRD), or end-stage kidney disease (ESKD), to improve their quality of life.

In this role, you’ll provide compassionate, personalized care primarily to patients with CKD and ESRD. Responsibilities include comprehensive patient care, maintaining up-to-date records, scheduling and managing follow-up appointments, and contributing to a patient-focused model that prioritizes quality of life and outcomes.

Roles and Responsibilities
- Conduct in-home and virtual assessments, including annual wellness exams
- Educate patients and families on resources for a healthier life
- Document patient histories, diagnoses, exams, and treatment plans
- Coordinate with primary care, specialists, and other services to ensure cohesive care
- Maintain quality standards in all documentation and patient interactions
- Deliver evidence-based care aimed at reducing hospitalizations and maximizing patient satisfaction
- Prescribe medications, order tests, and collaborate with Monogram physicians
- Help patients close gaps in care through preventive education and diagnostic scheduling
- Assist patients in accessing educational materials
- Participate in care team meetings to develop and review treatment plans
Position Requirements
- Bilingual (English/Spanish) preferred
- Basic Life Support (BLS) certification (company support provided)
- Active, unrestricted Nurse Practitioner or Physician Assistant license and DEA certification
- Ability to work independently with access to reliable transportation
- Proficiency with medical instruments, patient data management systems, and point-of-interview technology
- Strong verbal and written communication skills across patient and partner interactions
- In-depth knowledge of federal/state regulations, including HIPAA and OSHA
- Minimum of 2 years of direct patient care experience; managed care/IPA/health plan experience preferred
- Experience in annual wellness visits or similar comprehensive virtual/in-home assessments
