Rochelle, IL    Home Health Physical Therapist Competitive Compensation / Full Time Home Health Physical Therapist Position Overview: MLR is looking for a passionate and skilled Home Health Physical Therapist to join our committed team. In this role, you'll deliver in-home therapy services to help patients improve mobility, strength, and overall function. Key Responsibilities for Home Health Physical Therapist: •Patient Assessments: Evaluate patients' physical abilities and limitations in their home environment, developing customized plans to enhance mobility, strength, and functional skills. •Therapy Planning & Implementation: Design and implement therapy programs to improve movement, balance, and daily function, with a focus on meeting the patient’s individual goals and needs. •Patient & Caregiver Education: Offer training to patients and their families on exercises, home modifications, and techniques to improve safety and mobility. •Collaboration with Healthcare Team: Work alongside physicians, nurses, occupational therapists, and other healthcare professionals to provide coordinated and patient-centered care. •Documentation: Ensure accurate and timely documentation of treatment plans, patient progress, and outcomes in accordance with home health care regulations and standards. Qualifications for Home Health Physical Therapist: •Education: Graduate of an accredited Physical Therapy program. •Certification: Illinois Licensure in Physical Therapy required within 6 months of hire. •Experience: Experience in home health or outpatient therapy is preferred, but new graduates with a solid clinical background are encouraged to apply. •Skills: Strong interpersonal and communication skills, excellent time management, and the ability to work independently while providing high-quality care in patients' homes.
