As an RN Care Manager, you'll take on an exciting hybrid position, balancing in-person home visits with the convenience of remote coaching. Your mission? To support family caregivers and their loved ones by providing personalized care and guidance. No night shifts, weekends, or on-call duties, ensuring you get that perfect work-life balance.

Key Responsibilities:

Conduct home visits within your assigned area to assess safety, monitor health changes, and offer clinical coaching, including medication reviews.
Follow an established care plan and provide ongoing coaching through our digital app, offering resource referrals and best practice advice.
Build trusting relationships with caregivers to encourage positive changes and help reduce burnout.
Maintain accurate and timely documentation, ensuring compliance with standards and regulations.
Take on additional projects and duties as needed.
What You Bring:

An active Registered Nurse (RN) License in the state of employment.
At least two years of recent experience in direct care for elders or adults with disabilities.
Capability to travel within your assigned area (a valid driver's license and auto insurance are a must).
Experience in case management, care planning, and assessment, particularly in eldercare, disabilities, or complex medical needs.
Proficiency with common software and web-based applications, including MS Office and case management systems.
Bilingual skills are a plus and can influence compensation.
