Position Overview:
Greenlife Healthcare Staffing is seeking a Licensed Master Social Worker - Nursing Home Surveyor|Complaint Investigator to fill an opening with a Review Company in New York, NY.

Requirements:
~ Must have an NYS Social Worker license
~ Minimum two (2) years clinical experience in a medical facility, preferably a nursing home
~ Preferred 2 years experience performing nursing home surveillance activities
~ SMQT Certification preferred; must be able to obtain within 12 months of employment

Benefits:
~ $39 per hour salary
~ Mileage reimbursement
~ Flexible schedule
~ Temporary to permanent position

Responsibilities:
~ Participate in surveys or complaint investigations including allegations of abuse and neglect
~ Perform surveillance activities|tasks at nursing homes to determine compliance with Federal and State regulations
~ Produce written documentation and draft Statements of Deficiencies (SOD) within required timeframes
~ Assess compliance with Plan of Correction within required timeframes
~ Complete data entry in Federal and State reporting databases
~ Participate in State monitoring during Immediate Jeopardy situations
~ Attend required meetings
~ Testify in administrative hearings as needed
~ Adhere to DOH work schedules and company policies and procedures
~ Travel to onsite locations according to work schedule
