Annual Salary: $100,000 – $110,000

Your Impact:
Conduct intake evaluations and patient assessments
Coordinate and monitor interdisciplinary care planning
Maintain accurate MDS documentation and ensure compliance with regulatory standards
Lead the MDS process to support appropriate billing and reimbursement
Input and manage medical codes and ensure integrity of medical records
Qualifications:
Associate’s or Bachelor’s degree in Nursing (ADN|BSN)
Active New York RN license
MDS Assessor Certification required
1–2 years of experience in long-term care with MDS coordination responsibilities
Strong communication skills and attention to detail
