Position Highlights:

Full-time position (5 days|week) providing primary care in 2 skilled nursing facilities and 1 assisted living facility

Collaborate with interdisciplinary teams, facility staff, and leadership
Participate in care coordination initiatives and quality improvement


Job Duties:

Provide primary care rounding in SNF and ALF settings
Diagnose and manage acute and chronic medical conditions
Develop and implement individualized care plans
Conduct clinical case reviews and share best practices
Foster strong relationships with patients, families, and facility staff


Compensation & Benefits:

Estimated annual compensation: $210,579 – $271,579 (no cap on productivity income)

Candidate Requirements:

Board-certified or board-eligible physician
Experience in post-acute care preferred but not required
Active and unrestricted Delaware medical license (or in process)
DEA license with prescriptive authority
