Position Overview:
Join a premier academic medical center as an RN Case Manager|Utilization Review Nurse. Drive effective patient care transitions, ensure resource optimization, and lead utilization review and discharge planning for high-acuity patients. Collaborate across disciplines while maintaining compliance with EPIC documentation and regulatory guidelines.

Qualifications:
~ Education: ADN|BSN from an accredited nursing program
~ Licensure: Active New York State RN license
~ Certification: Current BLS certification (or willing to recertify before employment)
~ Experience:
~ At least 1 year recent (last 2 years) acute care hospital experience in a facility of similar size|complexity
~ Strong background in hospital case management within an acute care setting
~ Preferred: Emergency Department Case Management (ED CM) experience
~ Technical Skills: EPIC EMR proficiency
Key Responsibilities:
~ Perform utilization reviews for care level appropriateness and coverage compliance
~ Develop and coordinate discharge plans and post-acute care arrangements
~ Document all care coordination in EPIC
~ Ensure adherence to payer guidelines and regulatory requirements
~ Partner with Emergency Department teams for rapid and effective case management
~ Monitor resource utilization trends and suggest process improvements
