Position Overview:
We are seeking a compassionate Field Registered Nurse to deliver specialized nursing care for a five-star rated home care facility. This role offers autonomy, patient-focused care, and the opportunity to build meaningful relationships while ensuring top-quality outcomes.

Qualifications:
~ Associate’s or Bachelor’s degree in Nursing
~ Active NY RN license & ability to drive
~ At least 1 year of hospital experience
~ Proficiency in Home Care Home Base EMR preferred
~ Strong communication, compassion, and adaptability
Key Responsibilities:
~ Conduct patient assessments and determine eligibility for home care services
~ Develop and implement individualized plans of care in collaboration with families and physicians
~ Deliver skilled nursing care (wound care, injections, medication management, disease education)
~ Supervise home health aides and coordinate with interdisciplinary teams
~ Document all visits and care provided in HCHB EMR system
~ Communicate with physicians, update care plans, and participate in case conferences
~ Educate patients and families to promote better health outcomes
~ Ensure compliance with infection control standards and agency policies
