Qualifications

~ Active NY RN License; must drive and travel across Bronx & Westchester County

Job Description
The Field Registered Nurse (Coordinator of Care) oversees all aspects of patient care in the home. This includes assessments, care planning, documentation, communication with physicians, and coordination with other healthcare professionals. You’ll ensure continuity of care while maintaining compliance with agency and regulatory standards.
Key Responsibilities
~ Conduct home assessments to determine eligibility for services
~ Develop individualized Plans of Care with physicians and families
~ Deliver skilled nursing (wound care, injections, medication management, disease education)
~ Supervise and orient home health aides per regulations
~ Monitor patient conditions; report and adjust treatment plans as needed
~ Document care and interventions in HCHB within required timeframes
~ Coordinate with interdisciplinary teams (PT, OT, MSW, speech therapy, nutrition)
~ Lead case management, including interim orders, recertifications, and discharge planning
~ Educate patients and families on disease management and home care practices
~ Participate in case conferences, quality improvement, and mandatory in-services
~ Follow infection control and safety protocols
