Position Overview:
We are seeking a Pediatric Field Registered Nurse to provide compassionate, specialized care to pediatric patients in their homes. Qualifications:
~ Education: Associate’s or Bachelor’s in Nursing
~ Licensure: Active NY RN License & Valid Driver’s License
~ Experience: At least 6 months of clinical experience
~ Technical Skills: Proficiency with Home Care Home Base EMR
~ Soft Skills: Compassion, adaptability, strong communication, and family education expertise
Key Responsibilities:
~ Conduct home assessments and determine patient eligibility for care
~ Develop and implement patient-specific Plans of Care with families and physicians
~ Provide skilled nursing care, including wound care, injections, medication management, and disease education
~ Document all assessments and treatments in HCHB within 48 hours
~ Coordinate care with interdisciplinary teams and physicians
~ Monitor follow-up items daily and maintain accurate records
~ Participate in case conferences, recertifications, and discharge planning
~ Adhere to infection control standards and best practices
