Position Overview:
We are seeking a compassionate Registered Nurse (RN) to join a five-star Assisted Living Facility in Haverstraw, Rockland County, NY. This part-time role focuses on providing high-quality patient care, developing care plans, and ensuring compliance with facility standards.

Schedule:
~ Thursday: 7:00 AM – 5:00 PM
~ Friday: 7:00 AM – 5:00 PM
~ Saturday: 7:00 AM – 5:00 PM

Requirements:
~ Associate or Bachelor’s Degree in Nursing
~ Active New York State RN license
~ Minimum 6 months of clinical experience
~ Strong communication, organizational, and problem-solving skills

Key Responsibilities:
~ Conduct patient assessments and reassessments (OASIS tool as required)
~ Develop and implement individualized Plans of Care with families, physicians, and supervisors
~ Provide skilled nursing services, treatments, and preventive procedures as ordered
~ Monitor and report patient changes; update care plans accordingly
~ Teach and counsel patients and caregivers on care needs
~ Document all care per facility standards
~ Supervise and orient home health aides
~ Maintain compliance with infection control, confidentiality, and patient rights policies
