Position Overview

Qualifications
~ Education: Graduate of an accredited nursing program
~ Licensure: Active Registered Nurse (RN) license in Kentucky; CPR certification required
~ Experience: Prior home visitation experience preferred
~ Technical Skills: Proficiency in phlebotomy (blood draws) and accurate clinical documentation
~ Soft Skills: Strong interpersonal, organizational, and communication skills; comfort with community outreach is essential
Key Responsibilities
~ Conduct in-home blood draws for lead testing
~ Provide patient education and follow-up case management
~ Assist with special public health projects
~ Document care accurately in patient records
