Position Overview

We are seeking a highly motivated and experienced Registered Nurse to join our team as a Patient Care Manager in the Home Health sector, based in Roswell, GA. This leadership role is key to overseeing and ensuring the delivery of high-quality, compassionate care to patients in their homes.

Qualifications

~ Education: BSN degree preferred or equivalent clinical experience
~ Licensure: Current RN license in the state of Georgia
~ Experience:

Prior home health care or related clinical experience

Minimum 2 years of management or supervisory experience in a home care setting (preferred)
~ Technical Skills: Proficiency in electronic medical records and healthcare technologies; knowledge of home health regulations and standards
~ Soft Skills: Strong communication and interpersonal abilities; organizational and problem-solving skills; proven leadership and team management capabilities

Key Responsibilities

~ Lead and supervise a team of healthcare professionals to deliver exceptional patient care
~ Coordinate patient care services and develop individualized care plans
~ Conduct regular patient assessments and monitor care plan effectiveness
~ Ensure compliance with all home health regulations and standards
~ Collaborate with multidisciplinary teams to optimize patient outcomes
~ Educate and support patients and their families regarding conditions and care plans
~ Maintain accurate, timely documentation of patient care and progress
