Position Overview:
We are seeking an experienced and dedicated Patient Care Manager to join our team in Norwood. This role is critical in ensuring the highest standards of patient care within a home health setting. Qualifications:
~ Education: Bachelor of Science in Nursing (BSN) preferred
~ Licensure: Current and unencumbered Registered Nurse (RN) license in the state of practice
~ Experience: A minimum of two years' experience in a management or supervisory role within a home care or home health setting is highly preferred
~ Technical Skills: Strong knowledge of professional standards, Medicare Conditions of Participation, and federal, state, and local regulations
~ Soft Skills: Exceptional leadership, mentorship, communication, and interpersonal skills. Ability to serve as a liaison between patients, families, and staff
Key Responsibilities:
~ Manage and oversee patient care services, ensuring compliance with all regulatory standards
~ Lead, mentor, and support nursing staff, fostering professional growth and enhanced patient outcomes
~ Collaborate with interdisciplinary teams to develop and implement individualized patient care plans
~ Ensure accurate and timely documentation of patient care activities and outcomes
~ Conduct regular audits of patient records and care plans to uphold quality assurance
~ Act as a liaison among patients, families, and healthcare providers to ensure seamless care coordination
~ Engage in quality improvement initiatives and participate in ongoing professional development
