About the role:

• Full-time, permanent position
• Monday-Friday 8a-5p
• AGNP / PA will work in an outpatient setting seeing an underserved adult/geriatric patient population
• Adult Geriatric Nurse Practitioner/ Physician Assistant will be responsible for Primary Care services helping individuals manage their chronic conditions, preventative care, and patient education
• APP will join a busy practice that offers a collaborative, team environment

Background:

• Board Certified Adult Geriatric Nurse Practitioner / AGNP or Physician Assistant / PA
• 2 years of experience as an Adult Geriatric Nurse Practitioner or Physician Assistant required
