Work 4 – Ten hour days while seeing on average of 10 patients per day seeing high risk adult patients.

A rewarding opportunity to care for adult/geriatric patients in our Integrated Care Program. Focus on post-acute or higher risk patients to impact utilization and improve their quality of life. Physician led care team  includes nurse practitioners, case managers, clinical pharmacists, registered nurses, medical assistants, and social workers to provide complete care to their patients. Small patient panel size 

•             These are post-acute or high risk adult patients only. Quality time spent with the patients.

•             Physician leads a multidisciplinary health-care team

•             Outpatient, ambulatory care clinic setting.
