Your Role with the Company:
- Full-time position, Flexible schedule and work/life balance
- 60-90 min visits specializing in peri menopause, GI, regenerative and functional medicine
- Provider will see 3-5 patients/day, home visits, some clinic visits and telehealth
- Nurse Practitioner will build up to carry a patient panel of 225 patients
Background Profile:
- FNP / Family Nurse Practitioner, APRN, Adult Gerontology Nurse Practitioner
- License and Independent practice authority in Illinois
- 2+ years of Hormone experience required, functional medicine experience desired
