We’re seeking a compassionate and proactive Registered Nurse to join our clinical team as a Care Navigator, coordinating wellness plans and supporting client success through consistent, personalized engagement.
Position Highlights
* Manage a dedicated panel of clients, providing proactive wellness outreach and coordination.
* Collaborate with physicians, APPs, and wellness professionals to deliver seamless care.
* Document and track labs, screenings, and follow-up plans for continuity of care.
* Support infusion therapy needs and backup IV coverage as needed.
* Enjoy a collaborative, luxury care environment focused on holistic health.
