(Physician/MD qualifications required) Family Practice - Medical Director (Family Medicine or Internal Medicine) Post Acute Care - Oxford, NY - Southern Tier region, NY



Position Highlights:

Full-time medical director role in a post-acute care setting (4 days|week)
Flexible weekday rounding schedule in skilled nursing facilities
Provide direct patient care and clinical oversight
Collaborate with interdisciplinary teams, facility staff, and leadership
Serve as a mentor and resource to advanced practice clinicians
Represent the organization within the local post-acute care community


Job Duties:

Perform primary care rounding for short- and long-term patients
Diagnose and manage chronic and acute medical conditions
Guide nurse practitioners with clinical reviews and mentorship
Engage in quality improvement and care coordination initiatives
Participate in strategic leadership discussions and care planning


Compensation & Benefits:

Estimated annual compensation: $231,800 – $246,651 (productivity-based, no cap)

Candidate Requirements:

Minimum 1 year of primary care experience required
Experience in post-acute care a plus
Active and unrestricted New York medical license (or in process)
Active DEA registration with prescriptive authority (or in process)
