Location: Hauppauge, Long Island (Suffolk County, NY) | Tri-State Area Preferred.
The Opportunity:
A rapidly expanding private equity–backed Management Services Organization (MSO) specializing in ophthalmology is seeking a seasoned, C-suite-level Chief Operating Officer (COO) to lead enterprise-wide operations across multiple physician practices and ambulatory surgery centers (ASCs).
Headquartered in Hauppauge, Long Island (Suffolk County), the organization is executing a disciplined buy-and-build growth strategy and preparing for its next equity event. The COO will serve as the primary execution leader of the value creation plan — driving EBITDA expansion, operational standardization, and scalable infrastructure across the platform.
This is a highly visible leadership role with direct reporting to the Board and private equity sponsor.
What You Will Own:
Enterprise EBITDA & Performance Optimization:
Full accountability for enterprise EBITDA performance.
Margin expansion through labor efficiency, provider productivity, and ASC throughput optimization
Surgical block utilization and case flow improvement across ASCs
Cost discipline initiatives while maintaining clinical excellence
Multi-Site Operational Leadership:
Oversight of geographically dispersed ophthalmology practices and ASCs
Implementation of standardized operating models and KPI dashboards
Elimination of performance variability across locations
Development of repeatable operational playbooks
Private Equity & Investor Reporting:
Direct reporting to Board and PE stakeholders
Preparation and delivery of investor-level presentations.
Support upcoming equity partnership activity
Translation of operational strategy into measurable financial outcomes
M&A Integration & Platform Scale:
Lead operational diligence for acquisitions
Own post-close integration and synergy capture
Drive same-store growth across legacy practices
Support expansion initiatives across ASC and clinic footprint
Physician & Executive Alignment:
Align physician leaders around productivity and access standards
Balance autonomy with non-negotiable operational and financial metrics
Lead executive and site leadership teams with accountability and clarity
Required:
Current or former Chief Operating Officer (or true C-suite equivalent)
Experience within a private equity–backed, multi-site healthcare organization
Strong operational leadership across ambulatory or surgical environments
Demonstrated Board and PE reporting experience.
Proven ability to present to investors at a senior executive level
Preferred:
Ophthalmology experience (strong plus, not mandatory)
Ambulatory Surgery Center (ASC) leadership
Track record of EBITDA growth in a buy-and-build MSO model
Location & Travel Expectations:
Home base: Hauppauge, Long Island (Suffolk County, NY).
Candidates located within Manhattan, New Jersey, or Connecticut preferred
Full-time, on-site leadership presence expected.
Frequent travel to multiple ASCs and practice locations.
No relocation offered; overnight stays covered if within commuting distance.
Potential for partial remote flexibility after initial integration period.
Compensation:
Base Salary: $300,000 – $325,000
Performance-based bonus (negotiable)
No equity at present; potential future opportunity tied to new equity partner.
