Compensation & Benefits:

Responsibilities:
• Perform patient assessments, H&Ps, and care planning
• Manage acute and chronic conditions in post-acute settings
• Coordinate care with nursing staff and interdisciplinary teams
• Educate patients and families
• Maintain accurate and timely documentation
Qualifications:
• MD|DO with Massachusetts license and DEA (or ability to obtain)
• IM, FM, or Geriatrics background preferred
• Post-acute, inpatient, ED, or clinic experience welcomed
